Home Visits

Where possible we prefer to see patients at the surgery. However if you are unable to attend due to medical conditions we will visit you in your own home. Please try and contact us before 10am to arrange a home visit and be prepared to give full details to the reception staff. The doctor may telephone you back to assess the situation prior to visiting. House calls are usually carried out between 12.30pm and 3.30pm.

We are fortunate to have a team of volunteer drivers organised by our patient group who can transport patients to the surgery or hospitals if they do not have other means of transport. a small charge is made to cover costs - please ask at reception for details.

Requests for repeat prescriptions for regular monthly medication that have previously been authorised by a doctor can be made in one of the following ways :welton presc

  • Mark items required on computerised slip (right hand side of your prescription) and place in box adjacent to dispensary.
  • Complete slip and post to practice (enclose stamped addressed envelope if you want prescription posting back out to you).
  • Telephone the surgery and select repeat prescription from the options. You can then leave your request on the answer machine. This service is available 24hrs a day 7 days a week.

Minor Surgery

Appointments for minor surgical procedures can be arranged after consultation with the doctor.

Clinics

Chronic disease clinics for coronary heart disease, asthma, diabetes and hypertension are held throughout the week.

We also offer family planning services, smoking cessation, ante-natal and post-natal services, travel clinics (inc yellow fever) plus routine nursing clinics.

We also perform cervical smears - note this is a screening service not a diagnostic service and smears need only be performed every three years for ages 25-50 and every 5 years for ages 51-65. We usually contact patients to inform them that their smear is due put please contact us if you think yours is overdue.

Teenage Clinic

We offer help and support regarding all aspects of teenage health - lifestyle, relationship issues, smoking cessation, dietary advice, contraceptive services, stress management, drug and alcohol concerns. We also participate in the C-card scheme. Complete confidentiality is always maintained.

Test Results

Please contact the surgery between 2pm and 4pm for test results. Please allow 7 working days after the test has been sent before contacting us for results. If the test is very abnormal or treatment is required we will usually contact you  but we ask patients to contact us as a fail-safe. Please note we can only give results to the patient who had the test unless prior arrangement has been made to disclose information to a third party. We do not give pregnancy test results over the phone.

Medicals and Reports

We conduct private insurance medicals, employment medicals and other medical examinations. We are happy to complete forms for mortgage applications, holiday insurance etc but must give priority to NHS work so please allow at least 7 days for such forms to be completed. We make a charge for non-NHS work (as do all doctors). Charges are displayed in our waiting room and payment is required upon collection of the completed form.

Sickness Certificates

A sickness certificate is not legally necessary for the first 7 days of sickness - you need to self certify using the forms provided by your employer. After 7 days a doctor may issue a certificate of incapacity to work following a consultation with the patient.

Referrals

Sometimes patients will need to be referred to other services by the GPs for further investigation / treatment. Most referrals are now made electronically using the “choose and book” system - with some specialties you are able to choose the hospital you wish to attend and select from a list of appointment dates/ times. Private referrals will go direct to the consultant . Private referrals have no priority and are treated the same as NHS referrals.
