PDA Transport terms and conditions
This service is only for patients registered at the Welton Family Health Centre who have difficulty sourcing their own transport to attend medical appointments.
Examples of such appointments are; hospital, dentist, physiotherapy, medical clinics, scans, other specialist treatment clinics.
Requests for transport to visit relations in hospitals or care homes will be considered, but are dependent upon driver availability and given a lower priority to those directly attending medical appointments.
Do not use this service for transport to Welton Family Health Centre. Please contact the Health Centre directly for this option.
The PDA has a number of volunteer drivers who use their own vehicles in their own time to provide this service. A charge is made of 45 pence per mile from the driver’s home to the patient’s address then on to the destination and return, plus any car parking charges, tolls or other unavoidable charges incurred during the journey. This is payable in cash directly to the driver on the day of the appointment.
The transport scheme does not allow for shopping or other diversions, other than the collection of prescriptions.
Please give as much notice as possible in order for the coordinators to source a driver. Same day requests cannot always be managed but are considered if a driver can be found.
If children are carried that need a child seat, the parent or carer accompanying them must provide and fit a suitable and approved child car seat as per legislation.
No patient or passenger who are known to be suffering from a contagious or infectious disease will be offered transport.

The transport coordinator will arrange a driver and then telephone the patient with the driver’s name contact number and collection time.
It is the patient’s responsibility to contact the driver in the event of the appointment being cancelled or amended.
